121 Care – Disability Support Services

In 1992, a small group of people with disabilities realised their vision of creating an organisation to provide the daily supports they required to live a productive and purposeful life.

A fully accredited non-profit organisation, 121 Care provides support for people with a disability on the Sunshine Coast, South Burnett, Brisbane and regional areas. This includes providing practical support, services, advocacy, housing solutions, a public forum and use of an accessible vehicle.

All staff have a current criminal history check |  first aid and CPR |  manual handling training | workplace, health & safety training.

Under the guidance of the Management Committee, clients and their family members, the organisation has developed and grown into a small, robust service provider. We encourage people with disabilities to access educational, employment and social opportunities.

  • People with a disability have the right to the same opportunities, lifestyle options, level of security and dignity that are enjoyed by members of society who do not have a disability
  • We believe that for a society to be “just”, there must be a commitment made to support individuals with a disability regardless of the financial cost;  support to be “needs based”.

Providing support for people with disabilities

121 Care will make every reasonable effort to make the person with a disability aware of the standard of service that they can expect and that services will be provided in a safe manner which respects their dignity and independence and is responsive to their social, cultural and physical needs.

Access to the service will be decided on the basis of need and the capacity of the service to meet that need. 121 Care will take into account the client’s views in planning and evaluation of the service.

Eligibility criteria and Application

121 Care services are open to people with disabilities in South East Queensland who:

  • have allocated funding to pay for services
  • require services to assist them to live independently
  • wish to manage services on their own behalf or who have someone who can mange services on their behalf
  • are wiling to enter into the 121 Care Service Agreement and abide by the Code of Conduct
  • require service delivery in South East Queensland
  • request services that 121 Care staff are able to provide lawfully, safely and effectively
  • Eligibility is assessed by the General Manager based on these criteria.
